Peter Blos a mantenido la idea de un segundo proceso de separación e individuación a la adolescencia el cual ha sido pensado como un impulso obligado de la diferenciación psíquica que es provocada por las transformaciones pubertarias. Sin embargo, la fecundidad de esta hipótesis teórica merece ser interrogada teniendo en cuenta las exigencias de la metapsicología freudiana. Igualmente, parece importante tomar en consideración el papel que se otorga el sujeto a sí mismo sin olvidar que las respuestas de dicho objeto tiene una influencia sobre el desarrollo del proceso indefinido que es la apropiación subjetiva. Podemos también verificar el interés de esta hipótesis para la comprehensión de ciertas manifestaciones psicopatológicas durante la adolescencia, periodo que nos impone transformaciones específicas a la relación terapeutica.

Un tal titulo por lo menos enigmático impone el recurso a un diccionario para al menos entender algunas significaciones potenciales. Apoyándose sobre las diferentes definiciones propuestas, algunos polos de reflexión emergen para los clínicos que a veces intentan en vano de atraer y fidelizar los adolescentes en sus « fondos de comercio » Existe una expresión clínica de « el adolescente in consumible » al menos juzgado como tal por sus prójimos y que por lo menos se sustrae de la misma manera a toda forma de apetencia psicoterapéutica hacia el. La fantasmatica del sujeto/objeto consumible que representa esta estrategia de evitamiento merita de ser explorada. En las situaciones las más preocupantes. Ello viene a significar una adolescencia in consumible como rechazo de un cuerpo sexuado o del devenir adulto. La pregunta última reside en cual dispositivo psicoterapéutico tolerable para un adolescente sobre ese registro.

El adolescente in consumible, concierne una población numerosa de adolescentes que se encuentran bajo el yugo parental que abusan de su poder explotándolos a punto de negar el estatuto de sujeto. Entre esos adolescentes muchos de ellos caen en comportamientos de autodestrucción para proteger a los padres a quienes la violencia les esta fundamentalmente destinada. Otros escapan a esta obligación y logran preservar los procesos de subjetivacion. Ellos indican que hay soluciones posibles para ayudar a los primeros a volverse in consumibles.

El hechos crecientes de desordenes de comportamiento, de conductas a riesgo de apetencia adictiva, de depresión y de gestos suicidarios en los adolescentes significa una entrada brutal en la patología de los lazos y de la dependencia. Los políticos no son ni ciegos ni sordos a este problema y a las repercusiones vitales para el avenir de todos. Han tomado conciencia después de veinte anos haciendo del adolescente una prioridad de salud pública. Nuevas ideas han emergido de esta manera, liberando las esperanzas y las iniciativas innovantes pero también confrontándose a las dificultades y a los fracasos. La experiencia de una unidad de psiquiatría de adolescentes, nos muestra de una manera ejemplar, los limites de una política « reglamentada » en materia de tratamiento psíquico para adolescentes que se hallan en un registro « limite ». Las dificultades administrativas que están en relación con una reglamentación obsoleta o a recomendaciones nuevas que tienen por objetivo el de promover las nuevas practicas van a venir perjudicar paradójicamente su ejecución. Sin embargo no se sabría subestimar el valor defensivo de la labor y del discurso administrativo ni tampoco, el peso de ciertas corrientes de pensamiento que denigran los fundamentos psicodinamicos de la « cura relacional ». El riesgo podría estar vinculado a la irresistible hegemonía del « modelo tecnocratico » el cual se presenta como una nueva utopía ignorante de su inscripción en la corriente de pensamiento « positivista » y de utilización sutil que pueden hacer los poderes establecidos. Sin embargo no hay que dudar que los «psiquistas» a través de su intervención directa y su « practica intermediaria » (Rousillon, 1991) deberán proteger un espacio determinante en las instituciones de cura. Las cuales no podrán pasarse de su vigilancia frente a los « impensados » de la estructura y frente a las pasiones efímeras que las atraviesan como modelos defensivos a los cuales a los cuales uno acude.

Christian Mille : relevance of separation work in adolescence ?

Peter Blos has argued for the idea of a second process of separation-individuation in adolescence, conceived as an obligatory revival of psychical differentiation put in to difficulty by the upheavals of puberty. However, the fruitfulness of this theory deserves to be investigated in light of the requirements of Freudian metapsychology. It is also important to take into account the role reserved for the object itself, object whose responses are not without consequence for the development of the undefined process of subjective appropriation. Can we finally verify the value of this hypothesis in the comprehension of certain psychopathological manifestations in adolescence which require specific rearrangements in the therapeutic relationship ?

Such a title, which is enigmatic to say the least, requires us to resort to a dictionary in order to draw out some possible meanings. On the basis of the different definitions suggested, some lines of reflection open up for therapists trying, sometimes in vain, to attract adolescents to their « merchandise » and make them loyal customers. There is a clinical expression, « the adolescent not for consumption », or who is at least viewed as such by those close to him, and who removes himself from all forms of psychotherapeutic appetence addressed to him. The fantasy of consumable subject/object which underlies this strategy of avoidance warrants investigation. In the most worrisome situations, this fantasy will betray an adolescent who is not fit for consumption on account of his refusal of the sexually differentiated body or of becoming an adult. The ultimate question is that of what therapeutic setting can be tolerated by an adolescent in this register.

The « adolescent not for consumption » applies to a large number of adolescents struggling against parents who abuse their power by exploiting them to the point of denying their status as subject. Many of these adolescents succumb to self-destructive behaviors in order to spare their parents, the fundamental targets of their violence. Others escape this restriction and manage to preserve the process of subjectivation. They show that there can be solutions that might help keep the others from becoming consumable.

The increasing occurrence in adolescents of behavior disorders, risky conducts, addictive appetence, depression, and suicidal gestures signals an entrance into the pathology of bonds and of dependence. Politicians are not blind and deaf to this issue and to what is at stake, which is vital to the future of all. They have been aware of it for more than twenty years, making adolescence a public health priority. New ideas have thus emerged, giving rise to new hopes and innovative initiatives, but also bringing difficulties and failures. The experience of a psychiatric ward for adolescents shows in an exemplary way the limits of « regulating » politics when it comes to the psychical care of adolescents in a « borderline » register. Administrative restrictions tied to outmoded regulations or to new recommendations supposed to promote good practices paradoxically hinder their working. One should not underestimate the defensive aspect of administrative tasks and discourse, like the clout of some currents of thought which denigrate the psychodynamic bases of « relational treatment ». The risk may be linked to the irresistible rise of the « technocratic model » which presents itself as a new utopia, ignoring its inscription within the « positivist » current of thought and the subtle use that can be made of this by the powers-that-be. However, the « psychists », through their direct interventions or their « interstitial practice » (Roussillon, 1991), must undoubtedly retain a decisive place in the care facility, which can not do without their vigilance regarding the « unthought » aspects of the structure, the ephemeral passions that run through it and the defensive strategies it resorts to.

Christian Mille : il doit sortir ce soir : une urgence en pédiatrie

Dans une unité de soins pédiatriques la priorité absolue est bien de faire de la place pour les entrants potentiels, porteurs de pathologies somatiques relevant spécifiquement de la compétence du service: l’incontournable gestion des flux impose des choix et des décisions hâtives peu compatibles avec le tempo plus lent nécessaire à l’organisation des soins dans notre discipline. Cependant, bien que subsistent des divergences dans la temporalité des soins, le contraste tend à ne plus être aussi caricatural entre somaticiens et « psychistes », dès qu’ils sont engagés au quotidien dans des prises en charge conjointes. À un certain infléchissement possible de la culture pédiatrique championne des hospitalisations courtes, correspond sans doute le souci des partenaires « psy » de forger des arguments recevables  pour défendre, dans un tel contexte, des « indications » de séjour prolongé.

La fécondité de cette hypothèse théorique mérite cependant d’être interrogée au regard des exigences de la métapsychologie freudienne. Il importe pareillement de prendre en compte le rôle dévolu à l’objet lui-même, objet dont les réponses ne sont pas sans incidence sur le déroulement de ce processus indéfini d’appropriation subjective. Peut-on enfin vérifier l’intérêt de cette hypothèse dans la compréhension de certaines manifestations psychopathologiques à l’adolescence imposant des aménagements spécifiques de la relation thérapeutique ?
